The industry that keeps vulnerable people safe operates on systems that were outdated twenty years ago. Not because agencies don't want better tools, but because healthcare software has historically been expensive, complex, and built by developers who never talked to actual caregivers.

This gap represents both an opportunity and a responsibility. Building healthcare software means touching lives directly. Every bug might delay someone's care. Every usability improvement might help a caregiver focus on their patient instead of fighting their phone. The stakes are real in ways that most software development isn't.

The 21st Century Cures Act changed everything for home care agencies. Passed in 2016, this federal law mandated Electronic Visit Verification for all Medicaid-funded personal care and home health services. By January 2020 for personal care, and January 2023 for home health, every visit had to be electronically verified with specific data points: who provided the service, who received it, what service was performed, when it started and ended, and where it happened.

States that didn't implement EVV faced reduced federal Medicaid funding. Suddenly every home care agency needed technology they'd never needed before. Many turned to hastily-built EVV systems that checked compliance boxes but made caregivers' jobs harder. The opportunity remains for software that serves both regulatory requirements and actual human needs.

HIPAA adds another layer of complexity. The Health Insurance Portability and Accountability Act protects patient information with requirements that traditional software development rarely encounters. Protected Health Information—any individually identifiable health data—must be encrypted, access-controlled, and audit-logged. Violations carry fines up to two million dollars per year for repeated violations, with individual penalties reaching nearly seventy thousand dollars per incident.

HIPAA compliance shapes architecture from the start. Encryption at rest means database content is encrypted on disk—someone who steals the hard drive can't read the data. Encryption in transit means all communication uses HTTPS—no one can intercept data in flight. Project Setup and Architecture

Every decision in project setup reverberates through the entire codebase. Choose the wrong database structure and you'll fight it for months. Skip security considerations and you'll retrofit them painfully later. Healthcare software amplifies these stakes because the consequences of architectural mistakes affect patient care.

Traditional healthcare software development approached setup with extreme caution. Consultants spent weeks on requirements gathering. Architects debated schema designs. Security reviews blocked progress for months. This thoroughness had merit—healthcare data is sensitive—but it also meant projects took years to reach patients.

Vibe coding offers a different path. Not a reckless one—the sensitivity remains—but a path where architectural decisions can be made quickly, tested immediately, and revised if wrong. AI assistants know healthcare patterns. They can generate HIPAA-appropriate structures faster than humans can write requirements documents. The risk shifts from "did we plan enough" to "are we iterating on the right things."

The monorepo pattern suits healthcare platforms particularly well. A monorepo keeps all your code in one repository: the API server, the web application, the mobile app, shared libraries, database schemas. When you change a type definition, every package that uses that type sees the change immediately. When you update a security pattern, it propagates everywhere at once.

Healthcare platforms have natural divisions that map to packages within the monorepo. Core functionality—database access, authentication, types—lives in one shared package. The main API server orchestrates everything but delegates domain logic to specialized packages. Domain-specific features—care plans, scheduling, billing, compliance—each get their own package that depends on core but not on each other.

This separation matters because healthcare regulations treat different data types differently. Billing data has different retention requirements than clinical data. Compliance data needs different audit trails than scheduling data. Keeping these domains separate in code makes regulatory compliance more tractable.

TypeScript provides type safety that healthcare software particularly needs. A mismatched type in an e-commerce application might show the wrong price; a mismatched type in healthcare might administer the wrong medication dosage. TypeScript's compiler catches these errors before deployment, converting runtime crashes into build-time failures.

We configured TypeScript strictly for our healthcare project. Every variable must have a known type. Array access acknowledges that the element might not exist. Optional properties distinguish between "this field is missing" and "this field is explicitly set to nothing." These distinctions matter when you're dealing with medical records where missing data has different implications than zero values.

The database schema deserves careful thought because healthcare data has specific characteristics.

UUIDs work better than auto-incrementing integers for healthcare identifiers. Healthcare data frequently moves between systems—aggregators, clearinghouses, state registries. Auto-incrementing IDs collide during these integrations. UUIDs don't. The slight performance cost is worth the integration flexibility.

Separate tables for EVV records acknowledge that EVV has specific compliance requirements beyond normal visit tracking. A visit might be scheduled, completed, and paid without EVV. But EVV-required services need additional verification: location capture, signature collection, service type confirmation. A dedicated table for EVV compliance makes auditing straightforward—everything the state needs to verify lives in one place.

Audit logging must happen from day one, not as an afterthought. HIPAA requires knowing who accessed what protected information and when. An audit log table captures this systematically: user identity, action performed, entity affected, old and new values, timestamp, IP address. This logging feels verbose during development but proves invaluable during compliance reviews and incident investigations.

Geographic coordinates appear throughout healthcare schemas because location matters. EVV requires verifying that care was delivered at the patient's home, not somewhere else. This verification compares the caregiver's GPS coordinates at clock-in against the patient's known address. Storing latitude and longitude enables these distance calculations.

JSONB columns handle the semi-structured data that healthcare accumulates. Caregiver certifications vary by state, specialty, and agency. Rather than creating separate columns for every possible certification type, a JSONB column stores this flexible data. The same pattern works for aggregator responses, which vary by state EVV system.

Docker provides consistent development environments that mirror production. When every developer runs the same PostgreSQL version in the same configuration, "works on my machine" problems disappear. Docker Compose orchestrates multiple services—database, cache, application server—with a single command.

Security middleware belongs in the project setup, not sprinkled throughout later. Helmet configures security headers that prevent common web vulnerabilities. Rate limiting prevents abuse and denial-of-service attacks. These protections should apply to every endpoint automatically, not require developers to remember them for each new route.

The audit logging middleware intercepts every request that accesses protected information. When someone reads a patient record, the middleware logs that access. When someone updates a care plan, the middleware captures both the old and new values. This automatic logging ensures compliance without requiring developers to add logging calls to every function.

Environment configuration validates required settings at startup rather than failing mysteriously later. A healthcare application needs database credentials, authentication secrets, potentially API keys for aggregator integrations. Validating these requirements when the application starts—and failing clearly if they're missing—prevents the frustrating experience of discovering missing configuration during a critical operation.

When vibe coding project setup, we found that describing the overall architecture produced better results than requesting specific files. "Create a TypeScript monorepo for healthcare with separate packages for care plans, scheduling, billing, and compliance, with a shared core package for database and authentication" generates a coherent structure. Following up with specific requirements—"add HIPAA audit logging middleware" or "configure strict TypeScript settings"—refines that structure toward production readiness.

The setup phase is where AI assistance provides the most leverage. Generating boilerplate—package configurations, Docker files, TypeScript settings, database schemas—is exactly what AI excels at. These patterns exist in countless open-source projects; AI has seen them all. You can generate a production-quality project structure in minutes that would take days to write manually.

But setup is also where mistakes are most expensive to fix later. A database schema that doesn't support your actual workflows requires migrations and data transformations. A type system that doesn't model your domain forces constant workarounds. Security patterns that don't match regulations require auditing and retrofitting.

The technique we developed was iterative refinement of the foundation. Generate an initial structure. Build a simple feature on top of it. Notice what's awkward or missing. Refine the foundation. Repeat. This approach catches architectural problems while they're still cheap to fix, before the codebase grows around them.

Testing the foundation before building features prevents discovering problems after you've invested heavily in a flawed base. Can you run the database? Can you make authenticated API calls? Does the audit logging capture what you expect? These checks seem obvious but get skipped when teams are eager to start on "real" features.

The foundation work might feel like delay when you're eager to build healthcare features. But every hour invested in solid architecture saves days of debugging later. Healthcare software that works correctly matters more than healthcare software that ships quickly with hidden problems.

Care Plans and Tasks

Behind every home healthcare visit lies a care plan. This document—part clinical assessment, part authorization, part to-do list—determines what care a patient receives. Get the care plan right and caregivers know exactly what to do. Get it wrong and care becomes inconsistent, unauthorized services get delivered, reimbursement gets denied.

Care plans in home healthcare evolved from nursing documentation practices developed in hospitals. A care plan traditionally includes assessment of the patient's condition, goals they're working toward, interventions designed to achieve those goals, and specific tasks that implement those interventions. The document creates accountability: what was supposed to happen, what actually happened, how the patient progressed.

Before software, care plans lived in paper binders. Clinical staff wrote assessments in narrative form. Goals were stated as paragraphs. Task lists were handwritten and updated by crossing out completed items. This worked when agencies were small and care teams were tight-knit. It breaks down at scale. Paper gets lost. Handwriting gets misread. Different caregivers interpret the same narrative differently.

Software-based care plans promise consistency. A structured care plan defines goals in specific terms with measurable outcomes. Tasks have standard names and expected durations. Progress notes follow templates that ensure relevant information gets captured. Multiple caregivers visiting the same patient work from the same document, seeing the same tasks in the same order.

But structured care plans introduce their own challenges. Clinical staff find them constraining—real patient needs don't always fit predefined categories. Data entry becomes a burden when every observation must be clicked through dropdowns rather than described in natural language. The structure that enables consistency can also feel like bureaucratic overhead that distracts from actual caregiving.

The best care plan systems balance structure with flexibility. They provide templates and categories while allowing free-form notes. They enforce required fields where regulators demand them while staying out of the way for optional documentation. They translate clinical thinking into data without forcing clinicians to think like databases.

Understanding the hierarchy of care plan elements clarifies how to model them in software.

At the top level, a care plan belongs to a patient and covers a specific time period. The plan might be authorized for six months, with a certain number of hours per week approved by the payer. This authorization constrains everything below—you can't schedule more hours than are authorized, you can't deliver services that aren't on the plan.

Goals describe what the patient should achieve. "Improve mobility" is too vague; "Patient will walk fifty feet with walker independently by end of month" is specific and measurable. Goals have target dates and status tracking. A goal might be not started, in progress, achieved, or discontinued. Progress toward goals should be documented regularly.

Interventions are the clinical strategies for achieving goals. For a mobility goal, interventions might include range-of-motion exercises, supervised walking practice, and home safety assessment. Each intervention maps to specific tasks that caregivers perform during visits.

Tasks are the concrete activities—help patient walk to mailbox, perform passive stretching of affected limb, assess home for fall hazards. Tasks have categories like personal care, medication management, mobility support, nutrition, companionship. Different task types may require different caregiver certifications.

This hierarchy—care plan to goals to interventions to tasks—creates a traceable chain from high-level clinical objectives to day-to-day caregiver activities. When a caregiver completes a walking task, that completion contributes to the mobility intervention, which advances the mobility goal, which fulfills part of the care plan.

The data model we developed reflects this hierarchy with a few practical modifications.

Care plans store patient reference, date range, authorization details, and payer information. The status field tracks whether the plan is still a draft, actively being followed, or has been completed or cancelled. Linking to the payer enables downstream billing—we know who to invoice for services delivered under this plan.

Goals attach to care plans with their own identifiers and status. Target dates help track whether goals are being achieved on schedule. Progress notes attach to goals, creating a history of documentation about how the patient is advancing toward each objective.

Interventions connect goals to task templates. The intervention describes the clinical approach; task templates define the specific activities that implement that approach. This separation matters because the same intervention might apply to multiple goals, and the same task template might support multiple interventions.

Task templates aren't tasks themselves—they're patterns from which actual tasks are generated. When a visit is scheduled, the system examines the care plan's task templates and creates concrete tasks for that specific visit. This generation happens automatically, ensuring every visit has the right tasks without manual configuration.

The status lifecycle for tasks moves through predictable stages. Tasks start as pending when generated. They become in-progress when the caregiver begins work. They end as either completed or skipped, with completion capturing the timestamp and performer while skipping requires a reason. This lifecycle enables both real-time tracking and historical analysis.

One discovery that emerged from our vibe coding sessions: AI assistance works particularly well for care plan generation from unstructured input.

Clinical staff often know what care a patient needs but find it tedious to click through structured forms. They might naturally say: "Mrs. Johnson needs help with bathing and dressing in the morning, medication reminders twice daily, and someone to walk with her around the block for exercise." Translating this to structured care plan elements manually takes time.

AI can parse this natural language description and generate appropriate structure. Bathing and dressing become personal care tasks attached to an ADL (Activities of Daily Living) intervention supporting an independence goal. Medication reminders become medication management tasks on a schedule. Walking becomes a mobility task linked to an exercise intervention.

The technique that produced best results was providing AI with the care plan schema and asking it to generate structured data from descriptions. "Given this patient description, generate a care plan in this JSON format with appropriate goals, interventions, and tasks." The AI understands healthcare terminology and produces clinically reasonable structures.

This natural language input doesn't replace clinical judgment—staff review and modify the generated plans. But it dramatically reduces the data entry burden that discourages thorough documentation. Staff spend time thinking about care rather than navigating dropdown menus.

Task prioritization emerged as an important feature once we had structured tasks.

Not all tasks are equally urgent. Medication administration has timing constraints—a twice-daily medication needs to happen at appropriate intervals. Personal care should happen before mobility exercises because patients feel more comfortable exercising after grooming. Some tasks depend on others; wound assessment should precede wound dressing.

We implemented a prioritization system that considers multiple factors. Task category provides base priority—medication tasks rank higher than companionship tasks by default. Timing constraints add urgency as deadlines approach. Patient condition affects priority—a declining patient's tasks get elevated. Historical patterns matter too—if a task has been skipped repeatedly, subsequent instances get priority boosts.

The prioritization produces a ranked task list that caregivers work through in order. They can deviate when circumstances require—a patient who seems distressed gets companionship before scheduled activities—but the default order reflects clinical priorities.

Testing care plan logic requires understanding the domain.

A care plan can't be activated without goals—that's a business rule, not just a validation. Goals without interventions are clinically meaningless. Tasks without proper categorization can't be billed correctly. These rules emerge from healthcare practice, and the software must enforce them.

We tested these rules explicitly. Create a care plan without goals, try to activate it, expect rejection. Create a care plan with goals, activate it, verify success. The tests document the business rules as much as they verify the code.

Integration with scheduling comes next. Care plans define what should happen; scheduling determines when it happens and who does it. The scheduler examines care plans to understand service requirements, then matches those requirements against caregiver availability and skills.

A complete care plan system enables the workflows that home healthcare depends on. Clinical staff create plans that specify care needs. The system generates tasks for each visit. Caregivers complete tasks and document outcomes. Progress accumulates toward goals. Supervisors review and adjust plans based on patient response.

Caregiver Management and Matching

Caregivers are the human infrastructure of home healthcare. They travel to patients' homes, perform intimate personal care, make observations that clinical staff rely on, and often become the most consistent presence in their patients' lives. No amount of software sophistication matters if you can't get the right caregiver to the right patient at the right time.

Caregiver management has traditionally been one of home care's most painful operational challenges. Agencies juggle dozens or hundreds of caregivers with varying skills, certifications, schedules, geographic ranges, and personal preferences. Matching them to patient needs involves considering not just availability but compatibility—does the caregiver speak the patient's language? Are they comfortable with pets? Do they have reliable transportation to reach this neighborhood?

Paper-based caregiver management fails at scale. A coordinator might remember that Maria prefers morning shifts and John has a certification that expires next month, but these details slip through cracks when the agency grows. Spreadsheets help but become unwieldy. Purpose-built software promises to track everything systematically—but only if the system captures the right information and surfaces it at the right moments.

The caregiver profile serves as the foundation. Beyond basic contact information, the profile needs to capture everything relevant to matching and compliance. Certifications with expiration dates—a caregiver can't perform certain services after their CNA certification lapses. Languages spoken—critically important in diverse communities. Geographic service area—some caregivers drive long distances while others serve only specific neighborhoods. Availability patterns—morning person or night owl, weekday or weekend preferred.

Credentials management deserves particular attention because it has compliance implications. Home healthcare operates under state regulations that specify what credentials are required for different service types. A caregiver without a current tuberculosis test can't enter patient homes in some states. A caregiver whose background check hasn't been renewed can't work at all.

The traditional approach to credential tracking involves manual monitoring—someone reviews a spreadsheet monthly and calls caregivers whose certifications are expiring. This works until it doesn't. The coordinator gets busy, the spreadsheet falls out of date, and suddenly you discover that caregivers have been working with expired credentials for weeks.

Automated credential tracking transforms this liability into managed process. The system knows when each credential expires. It generates alerts at appropriate intervals—sixty days out, thirty days out, urgent notices in the final week. It can automatically change caregiver status when credentials lapse, preventing scheduling of non-compliant workers.

We discovered that credential management was an excellent candidate for vibe coding because the logic is clear even if the implementation details are tedious. "Track certification expiration dates and alert when approaching. Prevent scheduling caregivers with expired certifications. Generate compliance reports showing current credential status for all active caregivers." This prompt generates the tracking tables, alert logic, and reporting queries that would take hours to write manually.

Availability management sits at the intersection of caregiver preference and operational need.

Caregivers have lives outside work. They have children who need rides to school. They have second jobs. They have religious observances, medical appointments, family obligations. A scheduling system that ignores these constraints burns out caregivers and creates constant schedule disruptions when conflicts arise.

Structured availability capture lets caregivers specify their general patterns—available Monday through Friday mornings, not available weekends. Time-off requests handle specific dates. Preferred patient load settings prevent overwork. Travel limitations acknowledge that some caregivers don't drive or have geographic constraints.

The availability data becomes input to scheduling algorithms. When looking for someone to staff a Tuesday morning visit, the system first filters to caregivers available on Tuesday mornings, then considers certifications, geographic proximity, and patient-caregiver matching factors.

Patient-caregiver matching extends beyond simple availability checking.

The best matches consider multiple dimensions. Skill match ensures the caregiver can perform the required services. Geographic match minimizes travel time, which benefits both caregiver efficiency and patient reliability. Preference matching considers soft factors—patient requested a female caregiver, caregiver prefers working with dementia patients, language compatibility enables better communication.

Manual matching by coordinators works well when you have ten caregivers and twenty patients. It becomes impossible when those numbers grow to hundreds. The combinatorics explode—matching a hundred caregivers to two hundred patients across various time slots has millions of possible configurations.

AI-assisted matching can evaluate these possibilities systematically. We implemented a scoring algorithm that considers each matching dimension and produces an overall compatibility score. The algorithm doesn't make final decisions—coordinators retain control—but it surfaces the best candidates rather than requiring coordinators to mentally sort through every option.

The matching algorithm learned from historical data. When we tracked which caregiver-patient combinations led to continuity versus turnover, patterns emerged. Commute distance mattered more than we expected—caregivers assigned to distant patients left those assignments quickly. Language matching correlated with better patient satisfaction scores. These patterns informed the algorithm's weighting.

The caregiver mobile experience deserves consideration because it shapes daily workflow.

Caregivers spend their days in patient homes, not offices. Their primary interface with the scheduling system is a mobile app. If the app is clunky, slow, or confusing, caregivers won't use it—they'll write notes on paper and submit them later, creating data gaps and compliance risks.

We prioritized mobile-first design. Schedules appear clearly with all relevant information—patient name and address, expected tasks, any special notes. Navigation integration lets caregivers tap an address to launch directions. Clock-in and clock-out capture the EVV data points that regulators require.
